Suspended message when trying to log in

    I keep getting a suspended message when trying to log in. I have tried logging in with my username and my email. I have also reset my password and this still does not help. Also, the links to the forms I have created are taking me to a page that says the forms do not exist. Please let me know how to correct this problem.

    Upon review of your account I found that it was suspended due to form: requesting passport information on plain field texts. 

    Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our Terms of Use.

    If your purpose is not to collect identification numbers from the general public, but from your own customers, then you need to comply with two things.

    1. Add the Terms & Conditions Widget for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    I have reactivated your account, you can add both widgets to your form, if you also have other forms collecting this information you can add them as well in order to avoid any future inconvenience to your account.
