Cannot change form title

Hi

When I embed the form on our LMS - it has a title that I cannot see in its setting.

I do not know how to get rid of it.

Can you please share to us the URL link to your web page?

Kindly check your Page Title if it matches your Form Title.

Go to Settings>Form Settings>Page Title

To clarify, have you checked your form Page title that can be found going to your Settings>Form settings>Page Title?
