Dear management,
I cannot access my forms anymore because your system did not let me sign back. I was using your service for a testing purpose only. But I was thinking to upgrade to a paid subscription because your site was recommended to me by one of my friends. Can you please clarify my account.

Your account got suspended due to the password field to your form. We do not allow collecting login credentials here in JotForm. For more information, please read our terms of use.
I have deleted the guilty fields so I can re-activate your account immediately. You can now use your account and forms properly. Here's the screenshot of the deleted fields:

I do apologise not to read all the terms and conditions. I only did some tests. I will make sure not to use any guilty fields anymore.
