Received an error message which says hold it right there! My IP address has been banned because of phishing

    I have completed the form to notify Jotform there must be a mistake. As of yet I have received no response. Four potienal employees have attempted to utilize the application link,  all receiving the error message form not found, please help.

    This form was found on your account as suspicious of phishing activity because you're collecting copies of driver's licenses and social security cards.



    Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our Terms of Use.

    Our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.

    You may collect driver's license and social security numbers if your purpose is not to collect these data from the general public, but from your own customers, you just need to comply with two things.

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    However, we do not allow uploads of any sensitive personal information like passports, social security card, driver's license, credit/debit cards, bank statements, voided cheques, birth/marriage certificates, visas and photo IDs.

    We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending fields mentioned above.

    Your terms are agreeable and we would appreciate your quick removal of the suspension on our account so that we can resume business.
