My form has stopped working in Firefox. I can't be sure when it stopped, but I'm guessing it has been over the last two weeks. I havent changed anything my end. It still seems to work on other browsers.

I'm using iWeb, and I've tried all the suggested remedies such as using iframe code and also the proper iWeb code.

I can see it fine on my end but there are things you may consider fixing. The form is cut off in the right and at the bottom part. You cannot see the issue on MAC but in Windows OS, you can.

Change the value of height to 360px and width to 530px.
