Account suspended?

    I received the email about changing my form for security reasons. I didn't actually post it anywhere so I thought I'd have time to log on and make the required changes later. Today I attempt to log to make those changes and it's showing my account is suspended. Can this be resolved quickly?

    Upon review of your account I found that it was suspended due to form: http://www.jotform.us/form/83304263576155 requesting driver's license upload.

    You can request identification number, by adding the two mandatory widgets:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information. (Since you already have this widget on your form, you only need to add the Terms & Conditions Widget if you want to collect identification numbers.

    However, we do not allow uploads of any sensitive personal information like passports, visas,social security card, driver's license, credit/debit cards, bank statements, voided cheques, birth certificates, marriage certificates, death certificated and photo IDs.

    Upload buttons are not designed to protect sensitive information, so we suggest to refrain from using them with this type of data. In case of a security breach they are not going to secure the information at the servers.

    We have deleted the upload button and reactivated your account.

    Ok no problem but in order to remove those fields I would need to access the form by logging in. Which I can’t do until the account is unsuspended.

    I can see your account has been already un-suspended, please try logging in and removing the suggested fields.
