problem tracking form submissions on google analytics

Hi 

I have the following form on my website. https://miencarguito.com/new/formulario-de-registro/

once the clients register it takes them directly to a thank you page. https://miencarguito.com/new/gracias/

google analytics is not tracking the real source of where that client came from.how can i fix that?

Since the form is embedded to a web page, GA might be returning the page the form is embedded to instead of the referral page.  We have a widget that allows for getting the referral page:

https://widgets.jotform.com/widget/get_referrer

But I am not sure it would be possible to pass the same to Google Analytics.
