Why was my form suspended?  

It's a preliminary loan application, which is modeled after a sample from your site.

Upon review of your account I found that it was suspended because  form: http://www.jotform.us/form/82774234566162 is a mortgage application, since mortgage offers are only allowed to banking institutions and third party agents.
