Why is the Submit button appears in the incorrect place?

Hello, I have two issues with my form.  They are as follows:

1) Incorrect step count.

2) Submit button appears in the incorrect places.

I have checked your issue and found that few fields should be hidden to place the Submit button correctly. The step number also gets corrected. Please check the screenshot below.
