How to clear prepopulated sample values on form

for the following form - am seeing textbox -sample etc.. in the boxes t be completed.

how can i remove these please

It seems to be the autofill data. You may disable the autofill continue forms later option via the form settings.

Alternatively, try to submit the form. Once you submit the form it should clear the autofill values in your browser.
