I'm very happy to use your solution, but since 2 months, the mails sent with the forms doesn't send to mail reception, i have try with 5 forms but no reception.

Can you please configure your email alerts' sender name and email to "JotForm" and "email@example.com", respectively.
If changing Sender name and email does not help, ask your hosting provider to add these IP addresses to their whitelist:
