PDF attachments either blank or messed up

We are new to Jotform and using it on our Weebly site as of today.  the first pdf attachment came through fine with data on.  Since then, all of our test submissions have come through with just blank data, showing only the data fields, or garbled data with text on top of text.

We tried deleting the notification and starting over but it still didn't work.

Please advise what to do!

The formatting is correct in the PDF Editor. But I noticed that some text on top is not properly put into pages and is overlapping the PDF area. I've fixed that for you. Would you mind testing again?
