The PDF omits the heading section and gives an ID number instead.

    Hello, our caner charity is having problems with a new form - New Japan Patient Registration Form. The PDF omits the heading section and gives an ID number instead.

    Please advise.

    Try customizing the PDF and reset the template, see if that works for you.
