    Hi,

    Inject the following CSS Codes to your form. This should help fixed it.

    .form-matrix-table, .form-matrix-column-headers, .form-matrix-row-headers, .form-matrix-values{

    width: 60% !important;

    }

    Follow this guide: How to Inject Custom CSS Codes to your Form

    Let us know if you need anything else. Thanks!

    By the way, I was able to come up a better one. Please use the following codes instead:

    .form-matrix-column-headers{

    text-align:center;

    white-space: normal !important;

    }

    .form-matrix-table th {

    width: 500px !important;

    }

    This is how it looks on Mac - Safari.

    Thanks!
