I am embedding a form and it shows form over quota

Hi, in page

we are linking to a jotform as listed below

I emptied the form from previous years/files, but my site still shows the 'form over quota'

but I get a screen saying 'over quota'

Last year we payed $19 but we do not remember what that was for, if space or # of applications, and if it is one year or more.

Can you help us please ?
