How can I get my self grading quiz to issue the proper email notifying of passing grade?

My organization ran a quiz using the Jotform Quiz app in the fall and it worked perfectly. Based on their score, participants got an email certificate for PD credit. I used exactly the same formula, checked every detail and despite testing the Winter quiz with all correct answers, I get the failure message. I have spent days on this and am completely baffled and at a loss. I have compared the conditional statements, the settings and everything else between the fall and the winter quiz, yet the proper notifications are not going out. HELP!!!!!!!!!!!!!!!!!!!
