We have a website that we are very happy with with 1and1.com.  They recommended that we contact you for the interactive html format friendly format.

Is jotform.com the right direction for this.  If so, can you create the form and sent it to us for a fee?  The application is on our website (just downloadable .pdf now)

Firstly we just wanted to thank you for your interest in JotForm. Our forms are very html friendly and you can also do more advanced things with the form's source code as well. I do know that it is possible to create interesting and unique Employment Forms. You can see proof of this or even borrow and clone a copy to modify into your own version from our Form Template Gallery.

It is also fairly easy to create your own forms from scratch and we do offer several guides to get you started -

    (1) How to Create your First Form

    (2) Getting Started - More Tutorials

    (3) Email Notification and Autoresponder

Additionally if you want to see and understand what each current form field does you can see and read a more breif summary here -

    (4) Learning Form Fields

Or if it proves to still be troublesome to you just know that we are here to support you on a 24/7 basis if you happen to need any help from us and the best part is that our support comes at no cost to you! We keep the bar of highstandards raised to try to meet our customer's needs as best we can. The only costs that you incur here on JotForm is if you choose to upgrade your account to one of our Premium or Professional Plans and you can see and view the differences under Pricing or if you happen to want to use our JotForm Builder Engine as your Own on your Website.

Now as far as your last question goes could you please tell us more about where this application is? I think that i may have missed it on your site somewhere after looking around for a few minutes and we will try to help you recreate it. On top of that we do offer the ability to support pdf forms as well if that is what you are looking for.

I hope this sparks your interest more and if you have any further questions, problems or need more assistance with anything at all please do let us know.
