Entry field in the middle of a body of text

Hello,

We have used JotForm with great success for some of our forms, but I'm wondering if there are some options for greater customization, or if we could have a custom form created for us that we would still be able to edit.

The feature I am seeking is to have an entry field in the middle of a body of text.  For example, I would like to be able to create a form with sentences like the one below, with entry fields that within a sentence and do not break up the paragraph.

I, __________, am the parent/legal guardian of _____________, whose date of birth is __________.  I agree to the following terms and conditions, which shall begin on __________ and end on the day of discharge.
