Supplier Call Report 2019 - Live Jotform error occurring. HELP

I am trying to use this jot form I created and this error pops up every time.  I have changed browsers, networks, computers, tablet, phone, etc.  Why is this error occurring and how do I fix it? 

Try disabling the continue forms later feature and see if that helps, the option can be found on the form settings tab while editing your form on the builder:
