Login failed - SUSPENDED

    This is the second time my login has been suspended in 2 weeks because of "loan scam."  I'm trying to use this software to streamline the process of gaining new clients for my business.  You can see how this is frustrating, yes?

    Upon review of your account it was found suspended due to form: http://www.jotform.us/form/90006568639160 offering loans.

    A form offering loans must be done from authorized bank or credit unions only with company email address on their registered Jotform account.

    Kindly provide us these proofs :

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution.

    Send these proofs to reviewers@jotform.com with Thread #1697347  as your subject. After we review those, we will update you about the status of your account.

    By sending us those proofs will allow us to add your account to our whitelisted database.

    Nope not offering loans. I used the available template of a loan to create a client questionnaire. That template was a hassle so I discarded that project and started again with a blank form. 

    (If I were offering loans, I probably wouldn’t be using a free service to DIY it myself)

    Thank you for enabling my account. I’m happy to delete the loan template and carry on with my day.
