Selecting Multiple Options Using A Radio Button

    I need to allow users to select multiple options.

    Problems:

    None of the selected options are showing up in the submitted email.

    The form won't allow users to select multiple options.

    I tried using a check box but it still doesn't work.

    Black Tie Caterers Event Registration Form


    The "services" options are showing up in the form.

    When I view the submitted email it doesn't show the options that I selected.

    Hi,

    It seems to me you are referring to checkboxes (not radio options) which allows mutlple selection already -- on this form http://www.jotformpro.com/form/30256107721950

    Please correct us if this is not the form you are referring to.

    As for the email not showing the "services" option, I suggest you re-create the notification email (use the Add New Email button).

    Most likely, the form was edited/updated but the notifcation email was not. If you look on the right panel, the field name for "services" is multiple options.. not "service requested"

     

    When you create a new email, it will create all the necessary fields again.
