Form not displaying when viewed on iphone.

    Using latest ios version on iPhone 6s.

    Form works well on desktop and iPad

    This is a wordpress.org website, so I'm using the embed iframe code and not the responsive widget that I understand is deprecated.

    Kindly try changing the width in your iframe embed code to 500px. Check my screencast below.
