If we access the form directly then it works, but form is not loaded if accessed through the link on web site

If we access the form directly then it works 

But if we  click on the website link to go to the  form then form is not loading first time, if we refresh the page then it is loading

Hello, I have tested what you have shared, but I was not able to replicate reported problem. The form was loaded in 2 browsers I used for testing: 

Is your web site built on Shopify platform? If so, please try embedding the form according to this guide: Adding-a-form-to-Shopify-Store-Frontpage

But, I would recommend to first check once again if the form now works on your end, you may also try clearing browser cache to see if that helps.
