Form is not showing up on wordpress.com site

I have two forms I need on my website.  One has populated just fine and the other I can't seem to get to show up on my website.  I've tried on multiple devices and in multiple browsers.  

It has populated to this page just fine: https://osatransactionmanagement.com/open-a-file/

But doesn't work on this page: https://osatransactionmanagement.com/open-a-listing/ 

Please re-embed your form with the iFrame embed code to see if that will resolve the issue.

That didn't work either...

I tried deleting the page and rebuilding it and then adding the form and I also tried cloning the form and neither one works.

It appears that you are pasting the codes as a text based on the screenshot you provided.

Can you try if using the methods presented in this guide would help you solve the issue you are having, please?
