The .pdf downloading from the PDF Link on the Thank You page is blank

A .pdf copy of the filled form is attached to an email sent upon submission.  This .pdf is fine/has the necessary form data in it.

Can't upload a .pdf, apparently, so I just took a snip to attach.

Tested one of our other forms, PDF downloaded appears to be ok/correct.

Checked the .pdf email attachment layout (Bottom Pic).  It was blank, so I deleted it and created a new layout (and used the auto generated layout with minor edits).

Results: it updated the email received as an attachment, BUT if you click the PDF download link on the form submission Thank You page, the .pdf you download won't open. Acrobat just states ≈ "there was an error opening this pdf."

I have cloned your JotForm to my account and tested the PDF link displayed in the Thank you page. The PDF file downloaded correctly and the data is being displayed without any issue. Could you try removing the PDF link from the Thank you page and add it again to see if that works? You may also try using the following link:
