Data on Input Table not showing up.

Greetings:

I tried to make an edit earlier today in our survey form at https://www.jotform.com/build/82766488542168

I just tried adding a number to the column headers in the rating scale chart (Strongly agree, agree, etc.).  It then appeared that all of our hundreds of past submissions were then not populating that scale chart.   SO...I went to revision history and restored to an older version from November.  It LOOKED like the problem was solved, but new surveys coming in now are still coming in with that chart blank -- no bubbles filled in.  

Can you help us?  Just want form restored and the evaluation scale recorded for our participants. 

I can see the issue happened with some submissions, may they were sent at the same time you were performing the changes; however, the latest one seems to have received the data properly:
