Changing the title after I duplicate the survey?

Hi there,

I duplicated a survey called "rosacea", then I changed the title to be "acne", but when I go to "view form", the original "rosacea" title still appears.  Why is this happening?  How do I fix it without having to make a brand new survey?

You can change the title of the form by simply selecting the Form >> Click on More >> Rename >> Type the required name.
