I saw you had a note about issues with AT&T connections the other day (I am on an AT&T account) and was experiencing problems then, where there was no form on the page it was supposed to appear on. Yesterday, I checked and the form on my client's web page was working so I went ahead this morning and uploaded the new website to their live server only to find that the form is once again not working properly. This time I got a blank area where the form was to be and it took about 4 minutes to finally appear. I just went back to the page, clicked away to another page, then back to my form and once again it is missing.
What is the status and timeframe for correcting this problem? Or, is there something I need to do on my end?

We are truly about the issue. Just to let you know that we are still experiencing an access problem with jotform.us based forms. This issue is related to our Security Layer Provider, CloudFlare.
Initial solution is to clone the form (How to Clone a Form from a Web Page) and reembed it to your website. And try changing the form domain from jotform.us to jotform.com.
Just to let you know that the management is doing their best to resolved the issue as soon as possible without doing any changes to the form or doing the above solutions. We will let you know once this get fixed on this thread.
