How to revert to previous version of form?

Do you mean switch to the old form builder v3? If this is what you mean, it is not possible to switch on v3 anymore.

However, if you mean switch to the previous versions of your form, you can do that in the revisions history. Guide http://www.jotform.com/help/294-How-to-View-Form-Revision-History

I tried to do that but can't find the "Revision History" anywhere. I would like it to go back to my original format from like 6 months ago. Thanks

Just open your form and click on the "refresh" icon as shown below. This will populate all previous versions of your form on the right side of the page. Please refer to the screenshot below. You can select the version that you want to revert to.
