    When I receive an email from the form, some parts are missing.

    I have tried to re install the code, I've also re do the missing part but it doesnt change anything.

    I also tried to past the entire html code in dreamweaver but it gives the same results.

    Your e-mail notification alert for this form has a structure like this.

    As you can see all the areas which are assigned in e-mail notificator are filled in the screenshot you have sent. On the other hand there is some series of numbers in your notification e-mail structure. I believe they are the reason of gap in your mails.

    To create/update notification e-mails you can you open your form you desire click on "Setup & Embed" tab and click on "E-mail Alerts" button. There you can see Add New E-mail button and already defined notification alerts if there are any.
