     I need immediate access to the jotform account that was previously linked to username chughes. I was prompted by you to change my username to an activ

    I have checked the account username chughes and the account is in dormant status.

    The last form you have edited is under the account NCF_ORPS .

    You may use your email address to login to your account. If you have forgotten your password, you may follow this guide.
