    what is this for?  Please advise. This appears to be an unauthorized charge

    Please note that JotForm is an online form builder to create any type of forms to collect responses from your users. You may create any type of form viz., contact forms, surveys, application forms, order forms, sign-up forms, RSVP, etc using JotForm by using its intuitive drag and drop method in the form builder.     As I check our records, I am not able to find any accounts registered using your email address. However, I am able to find an email sent to your email address for the submission of the form https://www.jotform.us/90194340069153.
