I am getting spam emails from people

Hi , I'm not exactly sure how my name and address and phone number has been entered into this jotform database but I would like it removed immediately. I have been bombarded by spam emails of people wanting me to be their " assistants" and it's all fake and spammers. Can you please look into my account and remove anything that is associated with my email address : rdeoliv1@gmail.com. If you cannot do it, please inform me the necessary steps I need to take in order to remove my information from your system.

It looks like your email address is being used in the notification of fake job application form. The account is now suspended and your email has been removed. You will no longer receive those submissions.

Hi i am back again.

I got another email stating that "We have received your response for Job Application Form" I did not apply nor did I once log on to your website to apply for anything. Please remove any information about me from your database as it is an invasion of my privacy. You or whomever, has all of my information including my phone number and address which I did not consent on revealing to your website. I am tired of getting these JOTform emails and spammers emailing me mailing me packages to my residence.

Please make this stop immediately or there will be other actions taken .

I understand but I get emails from multiple people not just one person and it’s very annoying.... please remove me or block me entirely .
