Am I using the wrong fields to collect the time selection option?

    The time selection is not appearing on the report or form when checked. 
    Friday, April 5, 2019 @ 5:00pm and 7:30pm Squad

    Please clarify the exact issue. Are you not able to view the time selected in the email notification? 

    From your form, I can see that the below field is set up as a "Multiple choice" option. Since you require your customers to select only one choice, I suggest that you replace it with the "Single choice" option.
