    Can you un-suspend the SANDSTONE account so that we can modify the forms?

    Please email when available again and I will make updates according to your new TOS.

    These forms were found on your account as suspicious of phishing activity because you're collecting social security and driver's license numbers, ID upload, social security card upload and bank statements upload.




    Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our Terms of Use.

    You may collect identification numbers if your purpose is not to collect these data from the general public, but from your own customers, you just need to comply with two things:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    However, we do not allow uploads of any sensitive personal information like passports, social security cards, driver's licenses, credit/debit cards and statements, bank statements, voided cheques/checks, birth/marriage certificates, visas and photo IDs.

    We’ve removed the offending fields on your forms and lifted the suspension on your account. Please refrain from making new forms that is against our terms to avoid this kind of inconvenience.

    Hello. Is there a way to enable the two forms so that I can view the submissions? Neither of these forms collect information that violates Jotform TOS. Thank you.

    I have checked your account and it seems the suspension has been lifted. Your forms are active. You can now view the submission data in your form.
