I would like to fix this problem asap as my medical practice uses these forms from my website!
This is my second attempt at contacting you. The form just disappeared. Not sure where it went.

Please update the URL link of your Intake form which I found here
Since you are using secure URL of the form, you should correct the embedded URL link of your form to this https://secure.jotform.us/jsform/22597115986163
Please inform us if the issue persists after doing all this.
