Can you reinstate my account, its suspended

    I made some changes last night to the information forwarded via email when a form was filled out and submitted.

    My account is now locked

    This form was found on your account as suspicious of phishing activity because you're collecting social security and driver's license numbers.


    Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our Terms of Use.

    Our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.

    If your purpose is not to collect these data from the general public, but from your own customers, then you need to comply with two things:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    However, we do not allow uploads of any sensitive personal information like passports, social security cards, driver's licenses, credit/debit cards and statements, bank statements, voided cheques, birth/marriage certificates, visas and photo IDs.

    We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending fields mentioned above.
