I have a couple of questions on a form that has multiple choice answers in which the user needs to specify certain percentage(s) to each answer. Does anyone have any idea on the best way of setting this up? Thanks in advance for your help!

Please check on using Input Table field + Calculation
How to Perform Form Calculation in the Matrix Field
OR
Using the Grading tool widget.
I made a demo form you can test https://form.jotform.com/90727484222963
Another one is the Slider with Calculated Result Widget
The slider widget does not have input option, but it has slider thingy instead.
Let us know how we can be of further help.

Hi again, Jonathan,
I'm a bit dense, so I was able to create the form calculation in the matrix field but how do I get the '%' in there the way you did? And is there any way I can limit the answers to not go over 100% total?
Here's the form: https://form.jotform.com/90725876053160

You can do that in the condition section to get the value of each field in the input table.
Here's a screencast you can follow:
Related Guide: How to Insert Text or Calculation into a Field Using Conditional Logic
You can also use that to limit the value in each field.

Jherwin!!! You're teasing me. The screencast is way too fast. Anyway, I was able to that calculation but there are two things I am looking for:
1) Have the '%' symbol on the final output similar to Jonathan's sample;
2) Limit the total to 100%.
TIA!

Sorry about that. Here's the slower screencast with an instruction: https://www.dropbox.com/s/f9fifas6l0zud2x/Screencast.mp4?dl=0
You can also pause the video to make sure you're following the steps.

Thank you so much for that! How do I get the '%' symbol into the 'Total Percentage'? like this sample? https://form.jotform.com/90727484222963

You should add the '%' symbol into the calculation area.
I hope this helps!
If you need any further assistance, please let us know.

Hi Diana,
Thanks for your quick response. I copied every (almost) element in your sample except for the 'brown square' I can't figure out how to do it but still, it doesn't work.

Add an opening parenthesis at the beginning and a closing parenthesis at the end:

It is not working because you have this condition:
That condition is not needed, I have disabled that condition, so the calculation that shows the percentage sign works now in your form.

BDavid, thanks you so much for figuring it out. I was given that condition instruction previously. I appreciate your help.

I test your form and I can see now that the calculation with % symbol is now working properly.
As you have found already, there is a limitation on how the calculation work if getting the max percentage to be at 100% only at all times.
You cannot lock the result to be always at 100% similar to how the Slider with Calculated result work.
But what you can do is give an alert to the user if the total % is still below or above 100%. You can do it using calculation Conditions feature.
You can test it on my demo form https://www.jotform.com/90727484222963
