I was able to see the problem also when I test the PRINT button of the form embedded on your website. The PRINT button was not active when I tried using.

However this was not the case when I test using the form URL

The PRINT button on the form work as intended.

It seems the issue had to do with script conflict between your website's and those in the form.

What I suggest you do first is to re-embed the form on your website page.

Use instead the iframe embed code of the form.

Please try this first. Let us know if issue still persist.

I changed my website and embedded the iFrame code.
Print and clear buttons don’t work.
    Submit button works.

We apologize for the inconvenience caused. I test the iframe embedded form on your website and I was also able to encounter still the PRINT issue.

Please allow me more time to investigate what was causing the print problem. I will be back shortly.

Unfortunately I was not able to find a temporary fix. It seems there is a problem on the PRINT button itself.

I have now elevated the issue to the next level support for further investigation.

We will notify you here once we have fixed the issue.
