How to apply IF/THEN conditions to YES/NO questions?

    Hello


    I'm trying this out.  can you just see if I'm on the right track for getting the text and basics for now?  I just have done a couple of YES/NO but will need some IF/THEN boxes soon.

    I will also need to get signatures and make this HIPAA compliant eventually.  Am I in the right place to create a HIPAA form or does this need to be created in another area?

    Also how can I test this out on an ipad once this is done, and then submit this back to our home office to download as PDF?
