    I just signed up today but, when I try to login get "Login failed - AUTOSUSPENDED". I tried the 'Contact JotForm Support' page but it requires me to log in!!!

    Not a great start to my Jot Form experience. Please advise.

    The form http://www.jotformeu.com/form/90773185398370 was found on your account as suspicious of phishing activity because you're collecting user credentials.

    Gathering this type of data violates our Terms of Use.

    Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.


    We’ve removed the offending field on your form and lifted the suspension on your account. Please refrain from making new forms that is against our terms to avoid this kind of inconvenience.

    Thanks for your prompt response.

    This was my first form and was based on one of your offered templates.  Perhaps the potential for such an issue could be highlighted if an offending template is selected.  Might avoid a seemingly common problem.

    Upon checking, the form that caused the autosuspension was actually imported and not part of our templates gallery.
