Could you re-active my accounts?

    Having been unable to log in I thought possibly I had used the wrong password - but I've re-set it and stillo can't log in - the message says 'SUSPENDED'.  

    I haven't needed to use my forms for a while, but surely that shouldn't matter? 

    Please help!

    I've checked your account(username: Deanneasta) and forms' fields and your account is suspended because of violating our terms of use. I've deleted your passport numbers field and driver's license field because collecting (passport numbers, driver's license) information violets our terms of use. I've reactivated your accounts (Username: Deanneasta, Deanne). You can use your forms and accounts properly now.
