One of the page tab is missing

Our form no longer shows the 6th page in the tabs on top of the form (Payment Information).  I cannot find where there is a disconnect.  Your help is appreciated.

There are 6 pages in your Form and only 6 tabs are displayed in your Form.

If you want to add any further tabs then you would have to add more pages to your Form.
