Why my form is stuck on submitting?

I loaded a Membership Application form on my site, www.metrowestbusinesspartners.com.  After a user fills out the application, he receives a "Please Wait" indicator.  The Please Wait remains on the screen indefinitely and the form is not submitted.  

I followed the steps to embed the code on my wordpress site.  But the problem persists.
