Is there any suggestions for a read and initial section for forms?

wanting to add a read an initial portion to a form, I did a search and couldn’t find anything. Is there a widget or a way to do this like the attached picture?

You may simply add a text element with the desired text and then split your form in pages, leave the text element on the first page and when the users click on the "next" button they will have the form fields.
