My account was auto-suspended

    I was in the process of creating a form and it was suddenly auto-suspended.  I read the terms of service and was not asking for driver's license or social security information only family registration information. 

    Can you please let me know what the issue is?

    Your account has been suspended due to the password field you have on your form "MOPS FAll 2019":

    Note that asking for password is not allowed on JotForm, it's considered as phishing, we do understand this may not be the case with you,yet we need to act according to our terms.

    I have removed the field in order to reactivate your account, please kindly check and let us know if you have questions.
