Hippa vs regular

    I am the admin for a non-profit sports team.  I would like to have our medical release forms to be filled out online to avoid chasing athletes down for the forms.

    I'm not sure if what information I am collecting is necessary to be Hippa.  I also need to have a print out for out coaches in case of an emergency for contact info and 911. (we also have a release so we can secure reasonable medical treatment) 

    If I did a regular form, could I have a space for parents to sign that they understand this will be kept at our facility and will not be HIPPA compliant? 

    We collect dr name, medication list, tetnus shot and any pre-existing conditions that would effect their ability to preform the sport.

    We cannot advise as to whether or not you should use HIPAA compliant forms as we are not in a position to do so. We could say sure it looks fine but then something that isn't gets collected and we could be help liable. It is always recommended to do due diligence when dealing with potentially HIPAA protected data.  You can definitely collect the data and we do offer HIPAA compliant forms if you do decide the data being collected warrants the protection:
