My forms pop up and then disappear

When I login or click "my forms", I can see all of my forms for a moment and then they disappear quickly.  What is causing this to happen? 

I tried to log in your account, but I was not able to replicate the issue. Please try clearing your browser cache and cookies and see if that will fix the issue on your end.
