Clone feature not working for me

I completed and used one form, and I want to do a second form just like it. But when I clone the form, it looks nothing like the original form. The questions are the same, but the background, format, etc. are completely different. I tried to upload a screenshot here to show you, but your system said it was too large (which doesn't make sense). But you can look at my account to see the difference. 

I think you cloned a form and chose a classic layout. If you are going to clone card forms layout then you must choose card forms layout as well.
