Login Issues - Suspended Account

    I'm trying to log in to the account I have in JotForm, however, it's informing me that my login is suspended, I have forms created in my account and I'm not able to log in, how can I continue using JotForm. I await urgent contact. Thank you

    This form http://www.jotformz.com/form/90915912571662 was found on your account as suspicious of phishing activity because you're collecting identification upload.


    Online Form Builders, including JotForm, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our Terms of Use.

    You may collect identification numbers if your purpose is not to collect these data from the general public, but from your own customers, you just need to comply with two things:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    However, we do not allow uploads of any sensitive personal information like passports, social security cards, driver's licenses, credit/debit cards and statements, bank statements, voided cheques/checks, birth/marriage certificates, visas and photo IDs.

    We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending field mentioned above.
